Sensei Peacock expects you to: Just like any Martial Artist, Dance, and Fitness Instructor, to make a Commitment to your training. What specific things does she expect from a student? How to Demonstrate Commitment: *Show up to class regularly, and on Time (Live, Zoom, Pre Recorded Classes) *Work hard and focus while you are in class *Demonstrate Respect towards class mates and Instructor *Memorize (or commit to memory) certain sets of moves, forms, drills *See training as a long-term activity (It takes on average, 3 to 5 years to Master up to Black Belt, the art of NinTai, what effort is put in, is what you get out) *Use the lessons from class in your everyday life *You must show Honor

How do I show Honor? We seek to value something greater than oneself: We here at PureDefenseMMA live by a code of behavior. Violating that code is seen as betrayal of the group, and could bring punishment down on the individual (Such as Extra physical activity, probation, dismissal, and sit out time to evaluate ones behavior…

PureDefense expects you to show HONOR: (Examples given below) *Respecting the words and judgment of your teacher (even if you don’t agree, no back talking) *Taking action on suggestions/corrections that are made by your teacher (Take it as a learning expriance) *Not misusing martial arts/fitness/dance/other or abusing the physical power you develop (NO BULLYING or otherwise) *Not speaking badly about the school or anyone in it (NO GOSSIPING) *Being honest and open both inside and outside the dojo *Seeking to control your ego, act humbly and to speak with Kindness *Having good character inside and outside the school *Carrying on certain school traditions

(Please expect agressive/motivational/kind behavior, and loud/soft voice from Instructors, it's a part of training) We get excited for you, and it's our job to push you to be the best version of yourself.

STUDENT CREED AND OATH: I will conduct myself in a positive manner, and avoid anything that could reduce my mental growth or physical health. I will uphold the honor of Grace and develop self-discipline in order to bring out the best in myself and others. I will use what I learn in class constructively and defensively, I will never be abusive or offensive. I will uphold kindness, perseverance, self-control, and compassion. Class notes: I understand that I must acquirer gear & equipment approved by Rebecca Peacock. Due to COVID 19 we are no longer able to share gear: PureDefense Uniform Kit : (Logo Uniform Pants/Top, T-shirt, Belt, two patches, Mask) Uniform approved Mask:(Uniform grade Ninja mask required for all clubs, due to COVID required standards, until further notice. Arm/Hands & Leg Guards Mouth guard Shoes & Socks: (That you can run and jump in (NO BOOTS unless purchased Ninja Boots) (Ninja shoes can be purchased from club) (For Males) Male guard- required at every practice Upper levels need: Chest Shield Helmet with face shield Dance & Fitness club: Proper body coverings & gear issued out by class requirement

Few COVID notes: Must come dressed for class: No changing room available Large families please prep a place for potable potty in your vans for little ones: (They have bad aim, can not get clean enough for COVID requirement) We will be practicing outdoors: Bring bug spray, Outdoor sports weather rules. Appropriate underclothing: (No jeans and street clothes under uniform) Appropriate shoes: (Socks & Shoes that you can run, jump and get dirty in)

I Agree & Understand

In consideration of being allowed to participate in any way in (PureDefenseMMA any Clubs.class(es) run by Rebecca Peacock) athletic sports program, fitness, nutritional, dance, martial arts, bible study related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury, illness, mental and emotional sensitivity do exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of Rebecca Peacock immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS (Rebecca Peacock, PureDefense, PureDefenseMMA, Dance Club, Fitness club, any other clubs and/or products and/or training programs associated with Rebecca Peacock) their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I understand I participate at my own risk... and cannot hold any one in a court of law or fiscally responsible for anything that may occur. I understand I am responsible for myself. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
